My signature below indicates that I am adding my name to the Indian Hills Hole – in – one Club.    I understand that by adding my name to the Hole – in – One Club, a $5.00 charge will appear on my monthly Indian Hills statement when another member makes a hole – in – one.    If, for example, there are 200 members in the Hole – in – One Club and each one is charged $5.00, the member who got the Hole - in - one receives $500.00 ProShop Credit and $500.00 cash.
